How can you qualify for Patient of the Month Drawing?
What are the prizes? See below for details.

Patient of the Month Drawing

To qualify for the drawing, patients returning to the office for their 6 month check-ups and do not have any new cavities are eligible to place their names in for the drawing.

Drawings will take place at the end of each month and the winner will be posted on our web site beginning the first day of the following month.

The winner will only be announced on this website. If you see your name, contact our office at 667-3556 to claim your prizes. You only have until the last day of that month to claim your winnings.

 

PRIZES!

Prizes includes an individual prize for you, appropriate for age and gender, such as a large stuffed toy animal, computer game, sports, cards, etc.
